    2020 Update Form Adult :: This is the adult update form for 2020. If you have not been seen at the new location in 2020 you will need to complete this form and bring it to your appointment.

    2020 Update Form Child :: This is the child update form for 2020. If your child has not been seen at the new location in 2020 you will need to complete this form and bring it to their first appointment.

    2020 Controlled Substance Agreement :: This form should be completed on your first visit to the clinic in 2020.

Newly required CDS applications:

    CDS Application Adult :: This form is required under new laws that took effect in Nov of 2018. If you take a controlled medication please fill out this form and bring it to your first appointment.

    CDS Application Child :: This form is required under new laws that took effect in Nov 2018. These must be completed on or before the first appointment if a minor child receives a controlled medication.

Other Forms

    2020 Wellness Form :: This is new annual requirement from Medicare and other insurance companies. This is the yearly wellness screening for 2020. Dr. Schafer will start these in February of 2020 and you can fill them out and return these to the office on your next appointment.
